How To Request A West Virginia Crash Report

Start with the agency that investigated the crash, then choose the correct report request and keep related evidence separately.

Sources Checked October 4, 2026 General Information

A crash report request works best when it goes to the agency that actually investigated the collision. A crash that happened in West Virginia does not necessarily produce a West Virginia State Police report. Check the officer’s card, incident number, exchange sheet or correspondence for the department name. If a city police department or sheriff handled the investigation, ask that department about its records process before paying for a State Police search.

Find The State Police Crash Records Channel

For a State Police investigation, the Traffic Safety page links the Criminal and Crash Report Request Form. The shared title is easy to misread: the form handles different records, with separate fields and delivery contacts. Complete the vehicle crash section for a traffic collision. Do not substitute a criminal-record search or motor vehicle inspection inquiry simply because those services also appear on the agency website.

The linked Form 141, revised January 2024, asks for the crash date, location, drivers and report number. It also asks for the requester’s name, mailing address and contact information. Copy identifiers from the officer’s paperwork where possible. If you do not know a report number, provide the information you do have and ask Traffic Records whether it can identify the record before sending an incomplete paid request.

Match The Fee To The Requested Record

The form lists a $20 basic crime or vehicle crash report by mail and a $25 certified vehicle crash report. It separately lists fax delivery, reconstructed crash reports, photographs and video or audio. A reconstructed report is a different, much more expensive category; do not select it merely because you want an ordinary collision report. Confirm the category if an insurer or lawyer asks for a particular version.

Read the current form again before paying. The posted form states that cash, personal checks and business starter checks are not accepted and identifies permitted payment arrangements. It also describes charges for additional pages and separate media. A listed report price should not be treated as a promise that photographs, recordings, a reconstruction or every later supplement will be included. Keep a copy of the selected options and payment record.

Use The Address For Crash Requests

The form directs crash requests to the West Virginia State Police Traffic Records Section, 701 Jefferson Road, South Charleston, WV 25309. It provides a dedicated crash-report contact and asks mailed requests to include payment and a self-addressed envelope. Follow the address on the current crash form rather than assuming the general headquarters address or the criminal-record contact is interchangeable. Agency pages can serve different divisions.

Save the date you submitted the request, the delivery method and any confirmation or reference number. If the record cannot be located, compare the date, location, driver spelling and investigating agency with your original information. The form includes reasons such as incomplete information, incorrect fees or no available record. A request problem does not establish that no collision occurred, and ordering again without correcting the underlying detail may not help.

Review The Report And Identify Separate Evidence

Once the report arrives, check whether it describes your collision and whether names, vehicles, date and location match. Note factual discrepancies with the page number and the reason for your concern. Ask the investigating agency how it accepts correction requests or supplemental information. Preserve the original report and any agency response; do not alter a copy in a way that could obscure what the officer actually recorded.

The report is one part of a claim file. Scene photographs, witness contacts, repair estimates, towing invoices, medical records and insurer communications may be held by different people. The request form’s separate media options are a useful reminder that a narrative report does not automatically include every image or recording. Ask specifically about a needed item and identify who likely created or holds it.

Keep The Request Separate From Claim Decisions

Give an insurer the correct report number and explain if the copy is still pending. Keep your own notes of what you personally observed separate from conclusions you have not verified. An officer’s narrative may be useful evidence, but obtaining a report is not the same as resolving coverage, the amount of vehicle damage or a disputed claim against another driver.

If an insurance dispute develops, the state Property and Casualty Consumer Services division can investigate matters within its authority but says it cannot decide negligence or fault. Preserve the report alongside a clear timeline and the insurer’s written explanation. That combination is more useful for the next reviewer than a report alone, especially when the disagreement concerns something the officer did not observe.
